ProMotion Therapy is always looking for top massage professionals across the nation. If you have a passion for people and massage we would like to know. Fill out the application below so we can talk to you.

All applicants should have all necessary licenses, certificates and liability insurance. Also you must have all necessary equipment.(We will provide equipment for The Dade County Fair)

The Dade County Fair is coming and we need therapist. To join our team at the fair we will need the following information from you.

• Florida Massage License
• Proof of Liability Insurance. If you need insurance Click here.
• Completed W-9 form. To download Click here.
• Completed I-9 form. To download Click here.
• A back ground check. For an application Click here. Send your background check with this cover sheet here.
• An affidavit that states your back ground is clear and pledge to report any changes. To download Click here.
• Badge application (We must have all prior info to get badge.)

We will provide all the equipment needed at the fair, but we recommend you get your own as soon as you can. A date will be set soon for a short orientation and chair massage session. The date is to be set soon. Get us your information as soon as possible. We will be filling the schedule as quickly as possible on a first come first serve basis.
